Direct Debit Form (98KB) - Use this form to make extra regular contributions to your KiwiSaver account (for self-employed / not employed individuals). Employees can also use this form to make extra voluntary contributions direct to your KiwiSaver account.

Australian Superannuation Scheme Transfer Form (53KB) - Use this form if you would like to register your interest in regards to the transfer of funds from your Australian Superannuation Scheme.

New Zealand Superannuation Scheme Transfer Form (53KB) - Use this form if you would like to register your interest in regards to the transfer of your funds from a New Zealand Superannuation Scheme.

UK Pension Scheme Transfer Form (61KB) - Use this form if you would like to register your interest in regards to the transfer of funds from your UK Pension Scheme.

Permanent Emigration Application Form (63KB) - Use this form to apply for a withdrawal from the Huljich KiwiSaver Scheme on grounds of permanent emigration from New Zealand.

First Home Purchase Withdrawal Form (98KB) - Use this form to apply for a withdrawal of funds under the first home purchase withdrawal facility.

Determination for Previous Home Owner Form (114KB) - Use this form to apply for a withdrawal of funds under the home purchase withdrawal facility if you are a previous home-owner.

First Home Deposit Subsidy Form (114KB) - Use this form to apply for first home deposit subsidy with the Housing New Zealand Corporation.

Preferred Provider Form (55KB) - Use this form to select the Huljich KiwiSaver Scheme as your company's Preferred Provider of KiwiSaver.

Investment Option Switch Request (55KB) - Use this form to redirect your contributions to a different investment fund.

Prescribed Investor Rate (PIR) Form (53KB) - Use this form to change your Prescribed Investor Rate (PIR).

Direct Debit Amendment Form (61KB) - Use this form to amend your Huljich KiwiSaver Direct Debit contributions.

KiwiSaver Employee Information Pack KS3 (323KB) - Inland Revenue's KiwiSaver pack for employees.
